Terms & Conditions (Clinical Study)

1. Eligibility

I confirm that meets the study’s eligibility requirements as stated below:

• My dog is between 1 and 6 years old
• My dog is not currently taking any other pet supplements or medications
• I live in the United States

2. Study Commitments

In exchange for receiving over $900 in health tools and products at no cost (including shipping), I agree to:

• Feed POP-Topper daily for 90 days
• Submit two stool samples to Innovative Pet Lab (Day 0 and Day 90)
• Complete three owner surveys (Day 0, Day 45, and Day 90)
• Complete short weekly check-ins (3 questions, ~1 minute per week)
• Submit the Embark DNA test sample on Day 90

3. Use of Data

By participating, I consent to:

• POP, Innovative Pet Lab, and Embark collecting and using my dog’s data (e.g. stool test results, DNA insights, survey responses) to generate personalized health reports and support ongoing research.
• My dog’s samples and data potentially being used for future product development, internal research, or published study findings—without identifying me or my dog by name.
• POP securely storing my personal information (e.g. name, email, address) in accordance with applicable privacy laws. This data may be shared with study partners in the U.S. and New Zealand.

4. Additional Disclosures

• I understand this study is not intended to diagnose, treat, cure, or prevent any disease.
• No veterinary care will be provided as part of this study. If my dog shows any signs of illness or concerning symptoms, I will consult my own veterinarian immediately and record any visits as part of my weekly updates.
• I may withdraw from the study at any time without penalty. If I do, I may forfeit the $99 deposit.
• I understand that a refundable $99 deposit is required to participate. It will be returned in full upon successful completion of all study requirements.
• I consent to receiving email communications throughout the study and for up to 6 months afterward for follow-up questions or insights.
